Referring a Patient
The Congenital Heart Center is thankful for the work you do on behalf of the people you treat. We also recognize the importance of the relationships you share with your patients. That's why we're always working hard to make our referral process as simple and efficient as possible, so you can feel good about the care and treatment your patients receive.

The Congenital Heart Center is located within Children's Hospital of Illinois on the OSF Saint Francis Medical Center Campus.
Cardiologists are on call 24/7, so calls can also be taken after hours, on weekends and holidays.
Complete records are essential in determining the urgency of referrals. Please make every effort to have records sent to our office prior to the New Patient Appointment.
Records to be faxed include:
- Completed Specialty Request Form
- Copy of Insurance Card
- All pertinent test results
Follow up to referring physician office includes a phone call and letter with appointment date and time.
Physician's Access Line Service (Pals)
Our Physicians Access Line Service (PALS) is available to connect you with our subspecialty physicians 24/7. When you call the PALS line, one of our staff members will connect you by phone to the appropriate subspecialist.
